Check your confirmation and forms

After booking, find the confirmation email, including in your spam folder. Check the service, date, time, time zone and instructions for joining. A thank-you page alone does not confirm an appointment.

Complete the health forms and consent documents before the visit. If a question is unclear, ask for help rather than guessing. Use the instructions from the platform where you booked: IntakeQ for self-pay appointments or Headway for participating mental health insurance.

Keep your health information nearby

  • A photo ID.
  • Names and doses of current medications and supplements.
  • Allergies and previous medication reactions.
  • Relevant test results and treatment information.
  • A short list of your main questions.

If you do not know a medication’s name, keep its packaging nearby. Send documents only through the channel specified for your care.

Choose a private place in Florida

You must be physically in Florida during a Rio Breeze appointment. Find a quiet space where you can speak comfortably, charge your device, and test the camera, microphone and internet. Keep the device steady during the conversation. Do not join while driving.

If you are traveling or need someone to help you participate, contact the practice in advance to discuss arrangements.

Start with your main concern

You do not need a perfect explanation. Start with what prompted the visit, when it began and how it affects your routine. If you have several concerns, identify the most important ones and ask what can be covered in the appointment.

New clinical patients should choose the initial visit rather than a follow-up. Compare clinical visit types and prices before booking.

Leave with clear next steps

Before the conversation ends, ask about recommendations, any tests, follow-up and how to contact the practice. Write down what you need to do next and ask for clarification when something is unclear.
